Group Registration:
This form is ONLY for sponsors and camp leaders. Please specify the exact amount of students and sponsors which will be attending the camp from your church. For your group to be registered, Panfork must have the first and last name of each student and sponsor as well as the t-shirt size. This form is not optional and must be completely filled out.
Individual Registration:
If you are a student or a sponsor this form MUST be completed and submitted. For a student or sponsor to be registered and attend a Panfork camp this form must be completed, in full, and submitted by the required dates.
Family Camp Registration:
This form is only for Family Camp registration. If you are trying to register for any other camp please use the Individual or Group registration forms. If you are wanting to register for Family Camp please completely fill out this form.
